Hospital Transition & Discharge Service

Medically ready for discharge – but not yet ready to move forward?

For some people, being medically ready for discharge doesn't mean the right pathway out of hospital is ready.

The appropriate long-term living arrangement may not yet be available. Supports, equipment or funding may still need to be established. After an extended hospital stay, a person may also need time and the right environment to rebuild confidence, routines and everyday living skills.

ACE Supports provides a temporary, supported transition pathway between hospital and the person's next stage of life.

We work collaboratively with the person, their family or representatives, hospital Social Workers, NDIS Liaison Officers, Support Coordinators, Allied Health professionals and other key stakeholders to help address the barriers preventing a safe and sustainable discharge.

Our role is temporary. The outcome is long-term.

ACE Supports does not use the Hospital Transition & Discharge Service as a pathway into our ongoing supports.

Our role is to support the transition, work collaboratively with the person's broader team and help create the strongest possible foundation for their long-term living and support arrangements.

Because a successful discharge isn't simply about leaving hospital. It's about what happens afterwards.

Supported Independent Living

A home should feel like a home – not simply somewhere support is delivered.

ACE Supports provides individualised Supported Independent Living for NDIS participants who require rostered support to live as independently as possible within their home.

We believe successful shared living is about more than funding ratios.

Compatibility, choice, routines and belonging matter.

Where people are considering living together, we aim to involve participants and the people important to them in the process wherever possible.

This can create opportunities for prospective housemates to meet, build familiarity and have genuine input into the home and living environment they may share.

We're not simply looking to fill vacancies. We're working towards homes that have the best possible opportunity to work for the people living in them.

In-Home & Community Supports

Individualised support for everyday life – at home and within the community.

Everyone's everyday life looks different.

ACE Supports provides flexible, individualised assistance designed around the person's routines, abilities, preferences and goals.

In-Home Supports

Our In-Home Supports assist people to live safely, confidently and as independently as possible within their own home.

Depending on the person's individual needs, support may include:

• Personal care and daily routines
• Showering and dressing
• Meal preparation
• Household tasks
• Medication assistance
• Hoist and transfer support
• Building everyday living skills
• Assistance with appointments and health-related routines

We can also provide a range of participant-specific health-related supports where staff have received the appropriate training, competency assessment and clinical delegation or sign-off required for that individual.

Social & Community Supports

Community participation should be about more than simply getting out of the house.

We support people to explore their interests, build confidence, develop skills, connect with others and participate in activities and opportunities that are meaningful to them.

This may include building greater independence within the community, exploring new interests, volunteering, developing social connections, attending activities or working towards individual social and participation goals.

Wherever support happens, it starts with understanding the person and what matters to them.

Employment Assistance

Finding the right opportunity starts with understanding the person.

Employment assistance isn't simply about finding an available job.

ACE Supports works alongside NDIS participants to understand their strengths, interests and employment goals, build confidence and job readiness, prepare for interviews and explore opportunities that genuinely suit the individual.

Support may include:

• Identifying strengths, interests and employment goals
• Building confidence and job readiness
• Résumé and application assistance
• Exploring suitable employment opportunities
• Interview preparation and practice
• Workplace communication and expectations
• Supporting communication with prospective employers
• Preparing for the transition into a new workplace

We take the time to get to know the person – not just their résumé – so we're better placed to identify opportunities that suit them and help them present themselves confidently to prospective employers.

Support Doesn't Necessarily Stop When the Job Starts

Starting a new job can be one of the most challenging parts of the employment journey.

Where additional support would be beneficial, and with the agreement of the employer, ACE Supports can work alongside the participant during their initial days in the workplace.

This can help the participant become familiar with their role, understand workplace routines and expectations, build confidence and put into practice the instructions and training being provided by the employer.

It can also provide additional support during the initial training period, reducing some of the pressure on employers and their staff while the participant becomes more comfortable and confident in their new role.

The level and duration of this support is individualised and determined around the participant's needs and the workplace.

Our focus isn't simply on helping someone get a job. It's about giving them the best possible foundation to keep it.

We want to help create employment opportunities that are meaningful, sustainable and have the potential for long-term success.

Aged Care Supports

Supporting older Australians to live independently at home for longer.

ACE Supports has partnered with Trilogy Care, an approved Support at Home provider, giving people the flexibility to receive their care and support from ACE Supports using their Support at Home funding, subject to their individual care plan and allocated budget.

Trilogy Care administers the person's Support at Home funding, while ACE Supports provides the agreed care and support services.

Together, this partnership provides greater choice, control and flexibility over how support is delivered.

Depending on the person's individual care plan and budget, ACE Supports may assist with applicable supports such as:

• Personal care
• In-home support
• Assistance with daily routines
• Meal preparation
• Household assistance
• Medication assistance
• Support to attend appointments
• Other agreed supports within the person's care plan and budget

Privately Funded Supports

You don't need NDIS or aged care funding to access support from ACE.

ACE Supports welcomes privately funded individuals and families looking for flexible, individualised support outside government-funded programs.

Whether support is needed for a child, adult or older person, we take the time to understand the individual, their family circumstances, routines, preferences, culture and what is important to them.

We recognise that every person and family is different.

Where cultural, religious, language, privacy or gender preferences are important to the way support is provided, we will work with the individual and their family to understand and respect those preferences wherever possible.

Applicable ACE Supports services can be arranged and paid for privately, with support tailored around the person's individual needs and circumstances.
