Hospital Avoidance & Transition Supports

ACE Supports delivers safe, timely, and person-centred pathways for people leaving hospital or at risk of unnecessary admission. Our goal is to reduce hospital stays, support smooth discharges, and provide the right level of care during the critical transition back to community life.

We work alongside participants, families, and hospital teams to ensure every discharge is well-planned and supported. Whether the next step is returning home, moving into Supported Independent Living, or accessing Specialist Disability Accommodation, our team creates robust plans that keep people thriving and independent.

In 2025, ACE Supports, together with the South East Queensland Transitional Care Alliance, contributed a transitional care model to Queensland Health and the Department of Families, Seniors and Disability Services. This work reflected the practical resourcing needed for safe, cost-effective discharges and demonstrates our commitment to collaborative, evidence-based solutions.

By combining this system-level perspective with our personalised approach, we deliver culturally responsive, flexible supports that bridge hospital and community, reducing unnecessary hospital time and helping people move confidently into their next chapter.

Social and Community Supports (including Group Supports)

ACE Supports is passionate about helping people connect with their community, build friendships, and take part in meaningful activities. Our Social and Community Supports are tailored to each person’s interests, abilities, and goals.

This includes one-to-one support for things like appointments, shopping, or joining local activities, as well as group supports where participants can share experiences, learn new skills, and build lasting friendships in safe, supportive environments.

Whether it’s attending a community event, exploring hobbies, or developing life skills, we ensure every person has the opportunity to feel included, valued, and empowered.

Short Term & Medium Term Accommodation (STA & MTA) 

ACE Supports provides both Short Term Accommodation (STA) and Medium Term Accommodation (MTA) for people requiring safe, supported housing options during periods of transition.

 STA offers planned, short stays away from home that provide both respite for carers and new opportunities for participants, building independence, social connection, and confidence in daily living.

 MTA is designed for people who are waiting for longer-term housing solutions, such as Specialist Disability Accommodation (SDA), or where it is unsafe to return home immediately after hospital. We provide fully supported accommodation for up to 90 days, ensuring every participant has stability while permanent housing is arranged.

 Both STA and MTA are delivered in safe, comfortable, and fully supported homes, with 24/7 care available as required.

Community, Social and Recreational activities

We assist you with establishing community links, e.g. volunteering, employment, shopping, attending appointments, paying bills, social participation and maintaining friendships.

Individual life skills development, including public transport (travel training) and developing skills for community, social and recreational participation.

Improved living arrangements

We will guide and assist you to obtain/retain appropriate accommodation, this may include applying for rental tenancy or to undertake tenancy obligations.

Finding and keeping employment

We will provide on the job mentoring and training to ensure a successful work transition and placement, and we will assist and support employers throughout this transition.
