Hospital Transitioning Services
ACE Supports delivers safe, timely, and person-centred pathways for people leaving hospital or at risk of unnecessary admission. Our goal is to reduce hospital stays, support smooth discharges, and provide the right level of care during the critical transition back to community life.
We work alongside participants, families, and hospital teams to ensure every discharge is well-planned and supported. Whether the next step is returning home, moving into Supported Independent Living, or accessing Specialist Disability Accommodation, our team creates robust plans that keep people thriving and independent.
In 2025, ACE Supports, together with the South East Queensland Transitional Care Alliance, contributed a transitional care model to Queensland Health and the Department of Families, Seniors and Disability Services.
This work reflected the practical resourcing needed for safe, cost-effective discharges and demonstrates our commitment to collaborative, evidence-based solutions.

In-Home Supports
Supporting you to live safely, independently and confidently in your own home.
Home is where people feel most comfortable, and with the right support, many people can continue living safely and independently in the place they know and love. At ACE Supports, we understand that everyone's circumstances are different. Whether you're looking for support for yourself, a family member or someone close to you, we're here to help. We take the time to understand what matters most to you and tailor supports around your individual goals, preferences and daily routine.
Our supports are tailored to each person's individual needs and may include:
- Personal care, including showering and dressing
- Meal planning and preparation
- Household tasks and domestic assistance
- Medication assistance and administration in accordance with individual support needs, care plans and staff competencies
- Hoist and transfer assistance where appropriate equipment, training and support plans are in place
- Assistance with delegated health-related supports where staff are appropriately trained, assessed as competent and authorised
- Support following discharge from hospital to help people safely settle back into their home and daily routine
- Developing daily living skills and building independence
- Establishing routines that promote confidence, wellbeing and independence
- Assistance with attending essential appointments and maintaining everyday routines

Social & Community
ACE Supports is passionate about helping people connect with their community, build friendships, and take part in meaningful activities. Our Social and Community Supports are tailored to each person’s interests, abilities, and goals.
This includes one-to-one support for things like appointments, shopping, or joining local activities, as well as group supports where participants can share experiences, learn new skills, and build lasting friendships in safe, supportive environments.

Home & Living (ILO)
ILO isn’t about “filling a bed” or a one-size-fits-all housing model. It’s about creating a home life that feels right for you. With ILO, you choose how and where you live, who you live with, and the kind of supports you want around you.
It’s important to know: ILO does not fund the house or rent - it funds the supports that help make your living arrangement safe, sustainable, and independent. That’s why at ACE Supports, we don’t provide the housing. Instead, we walk beside you through two clear stages:
- Exploration & Design: We start by listening to your vision: where you’d like to live, who you’d like to live with, and what a good day at home looks like. Together, we explore options, consider informal supports (like family or friends), and design a plan that’s flexible and realistic.
- Supports & Monitoring: Once your ILO is in place, we deliver and coordinate your supports, check in regularly, and adjust as life changes. You’ll always have a voice in shaping how things work at home.
The NDIS recognises three funding levels (or “bands”) for ILO supports, depending on the amount and complexity of help you need. At ACE Supports, we explain what each level means in practice, so you understand from the start what’s possible, no jargon, no confusion.
What makes us different? Many providers misunderstand ILO by linking it directly to housing. We make it clear: you are in control of your home, and our role is to provide the right supports to help you live there with confidence and independence. Our focus is always on building skills so that, over time, you may need less support - not more.

Short Term & Medium Term Accommodation
STA offers planned, short stays away from home that provide both respite for carers and new opportunities for participants, building independence, social connection, and confidence in daily living.
MTA provides stability while waiting for permanent housing solutions. Both are delivered in safe, comfortable, and fully supported homes with 24/7 care available as required.
